An immigration exam is required by the USCIS (United States Citizenship and Immigration Services) to obtain a U.S. Green Card (or U.S. Citizenship).  At Health One Family Medicine, Dr. Parikh is a designated United States Civil Surgeon by the U.S. Department of Homeland Security and is authorized to perform immigration exams for all ages. We understand that the immigration process can be long and confusing - Health One Family Medicine provides all necessary medical services to complete your immigration exam in a convenient and timely manner.

What does the Immigration Exam include?

  • Physical Exam
  • Immunization history review
  • Screening for tuberculosis, syphilis, and gonorrhea
  • Signed and sealed I-693 form with all the exam results

If the required vaccinations are not completed at the time of visit, Health One is able to administer the vaccines during the first visit or refer out whatever it cost effective to patients.

 

What do I need to bring to the visit?

Please bring a valid government ID, such as a driver’s license or passport. You will also need a copy of your immunizations record.

 

Which immunization records are necessary?

Under the immigration laws of the United States, a foreign national who applies for an immigrant visa abroad, or who seeks to adjust status to a permanent resident while in the United States, is required to receive age-appropriate vaccinations:

  • Mumps
  • Measles
  • Rubella
  • Polio
  • Tetanus and Diphtheria Toxoids
  • Pertussis, Haemophilus influenzae type B
  • Influenza (during flu season)
  • Hepatitis B
  • COVID-19

If you have any records of current vaccinations, you should bring them with you, or we can refer you to the appropriate County Immunization Clinic or pharmacy.

 

Are immigration exams covered by insurance?

Most health and/or travel insurance plans do not cover immigration physical exams and vaccinations. It is an entirely self-pay or out of pocket visit.

In case you are missing any vaccines, we can provide the following vaccines/blood test at our clinic:

  • TDAP Vaccine 
  • Flu Vaccine
  • MMR Titer 
  • Varicella Titer 

 

Will I get a completed USCIS I-693 form upon completion?

Yes, our staff will give you a signed and sealed envelope which will contain the completed I-693 form. You should include this envelope in your USCIS immigration application.

At Health One Family Medicine, we value the health of every family member and want to make sure that our services are affordable for every patient. For that reason, we accept a wide variety of insurances. If you have any questions about the type of insurances we accept, please call our office. There will be a $25 charge for any missed appointments that are not cancelled 24 hours in advance.
